You will be asked some questions about your health. 

Things like:

-What food you eat

-If you smoke

-If you have any health problems, like epilepsy

-If you have problems with your eyes or ears

-You will be asked to have your blood pressure checked

-You might be asked to have a blood test.  This is where a small needle takes a small amount of blood out of your arm. 

-The nurse will check what medications you take and if they are up-to-date. 

If you are worried about anything above, please talk about it with the nurse.

The nurse will review your health action plan.
